Congratulations! You/your child has been accepted into the youth street dance training program KREATIV Crew!

By filling out the form below you officially accept your spot in KREATIV Crew for the entire dance year (fall 2026 - spring 2027), commit to the group's fees and schedules, as well as WEAREKREATIV's general terms and conditions. Our operations follow the principles of the safer space guidelines mentioned in the terms and conditions, so please make sure to familiarize yourself with them carefully.

The spot in the group must be accepted and the form below filled out by Friday June 5 2026 at 23:59.

NOTE! The guardian's contact information provided in this form will be used for billing and general information/ newsletter purposes. Billing will be done via email. Please add the emails of any other guardians in the "Additional information" box. These email addresses are used only for informing you about different schedules, events and other details regarding KREATIV Crew, the classes etc.
